The Outbound caller works as a member of a scheduling team to support Nurse Practitioners in the field who visit in home patients to provide health assessments. They contact the patient for scheduling the in‑home visit. They also assist their field team with finding the patient’s home, contacting patients, the field team cannot reach and other tasks that may come up on a daily basis. Call patients to schedule the in home visits for clinicians in the field Assist clinicians in resolving any issues or situations they encounter that require home office support. Surveyr patients concerning their in home visit to determine their satisfaction level and document any ideas for improvement.
