Health Lifestyle Coach - Telecommute with some travel
Health Lifestyle Coach The Health Lifestyle Coach provides nutrition education and lifestyle coaching to patients, working closely with the multidisciplinary healthcare team to help improve health outcomes for patients diagnosed with lifestyle-related chronic diseases related to diet and physical activity. Primary responsibilities include providing classes and individual coaching sessions to patients in-person or through telehealth that are evidence-based and patient-centered, managing the daily schedule to maximize patient care. Additional responsibilities may include assisting with projects, presenting on services to the healthcare team, and training new staff. Minimum Qualifications: Degrees: Bachelor's Degree in a health related field required (Degree in nutrition or dietetics preferred) Licenses & Certifications: Certified as a health or wellness coach through a National Board of Health and Wellness Coaches (NBHWC) approved program; required within six (6) months if held at time of appointment. Work Experience: One (1) year work experience providing nutrition education and/or lifestyle coaching to patients or in the community preferred Language Skills: Spanish skills preferred Communication Skills: Above Average Verbal Communication (Heavy Public Contact) Exceptional Verbal (Public Speaking) Writing/Correspondence Writing/Reports Proficiencies: MS Excel, MS Outlook, MS PowerPoint, MS Word, Personal Computer Job Attributes Knowledge/Skills/Abilities: Analytical, Mathematics, Medical Terminology, Research, Statistical Work Schedule: Flexible; Position works within the hours of the health center(s) that they cover. May include an occasional evening or weekend to cover off hours programming or a health fair. Telecommute; Hybrid Equipment Operated: Computer; culinary equipment related to food demonstration; weight scale Job Competencies: All required job competencies need to be completed within 90 days of hire.
