We’re seeking a skilled Physical Therapist (PT) to join our team and help patients regain independence through one-on-one home health care. Key Responsibilities: - Provide effective patient care using approved therapy techniques and procedures - Evaluate patient condition and establish measurable treatment goals - Develop and implement individualized treatment plans - Supervise and coordinate care with PTAs and HHAs - Document patient progress, updates, and treatment notes accurately and on time - Educate patients and families on exercises, mobility, and use of adaptive equipment - Evaluate home environments and recommend modifications to improve safety and independence - Ensure all services meet HIPAA standards for confidentiality - Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care Qualifications - Active Physical Therapist (PT) license in CA - Minimum 1 year of PT experience preferred - Physical exam within the last 6 months - Current TB test (within 12 months) and CPR certification - Valid driver’s license and reliable transportation Available Opportunities: We offer full-time, part-time, and per diem roles in AZ, CA, CT, FL, IL, IN, MA, MN, NV, and OH.
