Responsibilities

- Conduct initial reputed company assessments and home safety evaluations

- Coordinate care with physicians, families, and interdisciplinary team members
- Communicate professionally with clients, families, referral sources, and staff

- Mandatory Reporter certification
- Strong clinical assessment and documentation skills
