Key Responsibilities

• Utilize your education and professional training to conduct thorough psychosocial assessments, enabling you to formulate independent and accurate diagnoses for patients.
• Collaborate closely with patients to craft personalized, patient-centered treatment plans that address their unique emotional and mental health needs.
• Implement evidence-based interventions tailored to individual client situations, utilizing methodologies such as solution-focused treatment, behavioral activation, and motivational interviewing to drive progress.
• Coordinate ongoing communication and collaboration with our interdisciplinary team, including psychologists and primary care providers, to deliver integrated mental health care within the hospice setting.
• Monitor patient progress through systematic tracking of treatment responses, ensuring consistent evaluation of effective goals and adjustments as necessary, both in virtual appointments and occasional in-person visits.
• Assist with medication management as prescribed by medical providers by focusing on adherence to treatment plans, evaluating side effects and overall effectiveness to enhance patient outcomes.

Patient Population / Client Focus This position specifically focuses on serving individuals in hospice care. Clients may experience a range of emotional challenges related to terminal illness, requiring a compassionate and sensitive approach. You will be tasked with navigating complex emotional landscapes and supporting clients and their families during one of the most challenging periods of life.

Qualifications

• A Master’s degree in Social Work from an accredited institution is required to ensure comprehensive knowledge of mental health theory and practice.
• You must possess a current, valid, and unrestricted license to practice independently in mental health in the State of New York as a Licensed Clinical Social Worker (LCSW).
• A minimum of 1 year of experience as an independently licensed therapist is essential to effectively manage the responsibilities of this role.

Preferred Experience

• Experience working with hospice patients or within end-of-life care settings is highly desirable and will provide a deeper understanding of the specific needs of this population.
• Proficiency in virtual therapy modalities and experience using telehealth platforms to connect with clients will enhance your ability to provide care remotely.
• Familiarity with integrated care models and collaborative approaches to promoting comprehensive mental health care is beneficial.

Compensation and Benefits
