Key Responsibilities: Support administrative care management activities, including outreach and scheduling services. Serve as a point of contact for members, providers, and staff to resolve issues. Document member records in compliance with current state and regulatory guidelines. Key Requirements and Technology Experience: 2+ years of experience in a managed care environment or working with individuals with disabilities and/or vulnerable populations with chronic or complex conditions. High School diploma or GED required. Proficiency in Microsoft Outlook, Microsoft Teams, Zoom, Microsoft Word, Microsoft Excel, and SharePoint.
