Primary Care Registered Nurse - Home Health
Location Detail: 65 Commercial Blvd Torrington (10413) Shift Detail: PPV RN for the Torrington Territories, 8am to 4:30pm, 7 days per week and holidays as needed to support heavy PTO, LOA and busiest seasons Position Overview Territory: Torrington/ New Hartford Schedule: Full Time Monday- Friday, no regular call rotation Employment Type: Full Time Pay: Salary with mileage reimbursement Join Hartford HealthCare at Home as a Registered Nurse and bring high-quality, one-on-one care directly to patients in their homes. What You’ll Do Provide skilled nursing care to patients in home and community-based settings. Assess each patient’s condition, monitor disease progression, evaluate responses to treatment, and carry out the established plan of care. Educate patients and caregivers using practical teaching strategies that promote recovery, independence, and disease prevention. Coordinate care with physicians, Nurse Care Coordinators, therapists, LPNs, Home Care Aides, and other members of the care team. Maintain accurate, timely documentation in accordance with agency standards. Connect patients and families with appropriate community resources. Direct LPNs and supervise the Home Care Aide plan of care, as appropriate. Participate in ongoing education and professional development.
Qualifications
Graduate of an Associate Degree or diploma program in nursing. Current Registered Nurse license to practice in Connecticut. At least one year of nursing experience or one year of experience in a nursing specialty.
Preferred Qualifications
Bachelor of Science in Nursing from an NLN-approved program. Medical-surgical or home care experience.
